CARE SERVICES DIRECTOR
JOB DESCRIPTION
SUMMARY:
The Care Service Director acts as a patient and caregiver navigator to assist those living with ALS in Southern Michigan. This position serves as a liaison between patient/family, healthcare personnel, and the community to ensure care is provided to ALS families both promptly and effectively. This position reports to the Executive Director.
JOB RESPONSIBILITIES:
· Evaluate the ALS family to identify limitations which impede daily living.
· Work collaboratively as a liaison/team member between ALS Clinics to ensure effective communication and continuity of care for patient services.
· Encourage ALS families to routinely utilize all care service programs and services including: MyTurn, meals, communication program, loan closet, caregiver training and home visits.
· Plans and facilitates regional monthly support group(s) for clients/children/families, occasionally provide educational portion of monthly meeting, arrange for professional/expert topic speakers. Responsible for growth and function of support group.
· Conduct frequent home visits to ALS families in Southern Michigan.
· Maintain loan closet and ensure all equipment is accurately recorded and maintained.
· Monitor respite program and ensure all who need respite assistance receive the care and procedures are enforced.
· Supervise Care Service Manager and work with Care Service team daily.
· Work with Marketing and Development Manager to identify potential donors.
· Participate in fundraising events, community, and donor partnerships.
· Encourage third party fundraising to ALS families and friends.
· Writes donation recipients and thank you notes and letters.
· Administrative Functions – Maintains computer-based files of clients’ information and retrieval system for files and records. Prepares regular reports of activities of programs and services, tracks fundraising activities, ability to format and proofread detail-oriented letters, memoranda, and reports using Microsoft Office programs.
SUPERVISORY RESPONSIBILITIES:
· Carries out supervisory responsibilities in accordance with the organization's policies.
· Responsibilities include interviewing, hiring, and training employees, appraising performances, and resolving problems.
QUALIFICATIONS:
· Master of Social Work from an accredited MSW program
· Ability to assess both physical and psychosocial needs of people with ALS and their families.
· Knowledge of community healthcare resources.
· Strong organizational and communication skills, including computer skills.
· Ability to function independently within guidelines.
· Skill and experience in working with volunteers and students.
· Ability to make decisions, resolve conflicts and manage stress.
· Ability to maintain confidentiality of patient and family information.
· Flexibility to work evenings and weekends as needed.
· Flexibility to travel as needed.
· Five years of related professional experience, including direct care, and leadership or management experience, program design and project management.
· Strong business acumen with demonstrated analytical and problem-solving skills.
· Superior written and oral communication skills, including public presentation/professional in-service presentation experience.
· Ability to successfully identify and cultivate relationships with a wide range of stakeholders.
· Ability to contribute value to team projects, individually and as a group member.
